How does telemonitoring work for general practitioners?

As the primary treating physician (PTP), you can refer your patients to a Telemonitoring Center (TMC), where their health data is reviewed daily. The inCareNet HF dashboard and quarterly reports provide you with an overview of your patients’ condition. Automatic notifications alert you as soon as the TMC has forwarded a task to you.

What criteria must patients meet to participate in the telemonitoring program?

  • Heart failure in stage NYHA II or III with a left ventricular ejection fraction (LVEF) of < 40%

  • Guideline-based treatment of the existing heart failure

  • Exclusion of possible factors that could interfere with data transmission or self-management

  • Implant patients: carriers of an ICD or CRT system

  • Patients with external measuring devices: inpatient treatment of heart failure within the last 12 months
