As the primary treating physician (PTP), you can refer your patients to a Telemonitoring Center (TMC), where their health data is reviewed daily. What criteria must patients meet to participate in the telemonitoring program?

  • Heart failure in stage NYHA II or III with a left ventricular ejection fraction (LVEF) of < 40%

  • Guideline-based treatment of the existing heart failure

  • Exclusion of possible factors that could interfere with data transmission or self-management

  • Implant patients: carriers of an ICD or CRT system

  • Patients with external measuring devices: inpatient treatment of heart failure within the last 12 months
