What is telemonitoring?

Telemonitoring refers to the remote monitoring of health-relevant data and complements the existing treatment of heart patients.                                                                                                

If you have an implant (CRT-P, CRT-D, or ICD), routine check-ups (follow-ups) can also be performed remotely, giving you greater flexibility in your daily life. At the same time, your doctor can gain insights into your heart and implant data between follow-up appointments, enabling them to respond promptly to any changes. For the remote transmission of your implant data, you will receive a transmitter, e.g. the BIOTRONIK CardioMessenger.

If you need closer monitoring of your heart data—such as in the case of heart failure—your doctor may recommend daily telemonitoring. inCareNet HF is designed for this purpose and includes daily monitoring of key health parameters under medical supervision.

Your health insurance may cover the costs.

If your doctor prescribes telemonitoring, your health insurance may cover the costs for both daily monitoring of heart failure and routine remote follow ups.1

How can I participate in the telemonitoring program in Germany?

There are two ways to participate in the telemonitoring program:

If you have an implanted ICD or CRT system, you will receive a special patient device for home use. This device enables the regular transmission of health data from your implant to a telemonitoring center (TMC) supervised by medical specialists.

If you do not have an implant but have been hospitalized for cardiac decompensation within the past 12 months, you will receive a set of external measuring devices. With these, you can regularly record your body weight, blood pressure, heart rate, heart activity (ECG), and current well-being at home — and transmit the data digitally.

How does telemonitoring work for implant patients?

The data from the ICD or CRT device is automatically and event-based transmitted from home to the responsible telemonitoring center via a transmitter, where it is reviewed.

The only thing you need to do is place the transmitter next to your bed at night – the data is transmitted automatically. This allows your doctor to receive and analyze your health data promptly and make any necessary adjustments to your therapy.

How does telemonitoring work for patients with external measuring devices?

Your attending physician will provide you with a scale, a blood pressure monitor, a mobile ECG recorder, and a patient tablet. Using these devices, you can record your weight, blood pressure, heart rate, ECG and overall well-being from home every day. The collected data is transmitted securely and in compliance with data protection regulations to your telemonitoring center. The measurements take only a few minutes each day. With this minimal time investment, you help ensure that relevant changes in your health condition are detected early, potentially preventing complications such as hospitalizations.
